Terms and Conditions of Service

Welcome to the Churchill Center digital network. By accessing our specialized landing pages and initializing our assessment funnel, you agree to be legally bound by these Terms of Service. Please read them carefully.

1. Scope of the Initial Diagnostic Package

Your investment of $499 initializes a specific, limited, one-time service wrapper consisting explicitly of:

• One (1) specialized, multi-marker laboratory blood draw requisition.
• Laboratory processing and biomarker calculation.
• One (1) private, 45-minute diagnostic roadmap review session with Dr. Churchill.

Purchase of this package does not constitute or guarantee enrollment into Churchill Center’s long-term ongoing medical protocols or monthly care programs. Continued treatment beyond the initial 45-minute roadmap session requires a separate evaluation, mutual agreement, and a distinct financial agreement.

2. Payment & Billing Framework

All payments ($499) are securely executed and initialized via our integrated EHR merchant channel. Transactions will appear on your financial statements as Churchill Center. This service is explicitly cash-pay and is not billed directly to health insurance networks, though it is fully compatible with most Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA).

3. Cancellation, Rescheduling, and Refund Policy

Because our clinical team pre-allocates laboratory assets and coordination resources upon your submission, the following strict terms apply:

• Full Refund Eligibility: You may cancel your diagnostic package or reschedule your lab requisition up to twenty-four (24) hours before your scheduled appointment time for a complete 100% refund.
• Late Cancellations & No-Shows: Cancellations requested less than 24 hours prior to your scheduled slot, or failure to show up for your locked calendar appointment, will be subject to a non-refundable $150 processing and logistics fee.
• Geographic Availability: If our medical coordination network determines that we cannot service your submitted ZIP code, your transaction will be cancelled automatically, and an immediate 100% refund will be processed to your card on file.

4. Limitation of Liability

Churchill Center and its clinical staff are not liable for any technological delays or connection drops experienced during your online video consultation or form transmissions. We reserve the right to refuse service or cancel client intakes at our clinical discretion if a candidate does not meet medical onboarding requirements.
